I believe the underlying problem is a lack of self esteem. So that's what you should really be working on. Here are some ideas that you may not have considered.
Try doing some volunteer work, something that makes you feel valued and important. It might be working in a soup kitchen, or giving your time to a nursing home or hospital. If it involves speaking with people, so much the better.
Try attending a good church every Sunday, one where you can feel comfortable and get involved in social activities.
And look for ways to do public speaking on a regular basis. You could volunteer to do safety lectures at schools, or speak to elderly folks about Medicare.
It doesn't really matter what you are lecturing about or teaching. The experience should be helpful — especially if you are providing info that the group desperately needs!
